Proteus .4 Lowside Drivers dying.

I have had several low side drivers die in the last two weeks. As they were dying I was making changes to try to fix/troubleshoot the situation. I checked Resistance on all injectors, check the wires then ran all new wires to all 4 injectors (wired individually but set to batch fueling). Swapped out injectors with brand new Deatschwerks Bosch EV14 injectors. During this time the Lowside for the boost control mac valve also died. In the log that I have attached here you can see the Lambda go to full rich when the driver dies and locks the injector open at 977.702s. This particular time it was Lowside 4 on injector 3.
I have moved the pins driving the injectors and boost control as they were dying during the troubleshooting. I cannot figure it out so I thought I would put it up here. Nothing has changed with wiring or the car in a while. I have a backup proteus .3 that I could do more troubleshooting with if need be. Do you have any ideas or something else I can try.

When looking through logs from my test drive yesterday I see Warning 6711 (The CUSTOM_PWM_DUTY_TOO_HIGH = 6711) is occurring throughout the log. I cannot find which PMW this error is associated. I was wondering if this could be tied to the driver issue. Is there a way to see what could be causing this error? I went on a 40 minute test drive yesterday after swapping drivers and had no issues so far. If the weather holds out i will be testing again today. I would like to eliminate all possibilities for the cause of this issue.
